Technical Notes 41 Table 3.04 30 Introduction This report provides health status information for the year 2009 for residents of Maricopa County, Arizona. It includes data for mortality (death), communicable diseases, and natality (birth), as well as information on the size and demographic distribution of the population. For each disease or health indicator, the following information is provided: Number of cases in the county Mortality Disease Example: 5,420 residents of Maricopa County died due to cancer in 2009.* Communicable Disease Example: 65 residents of Maricopa County were diagnosed with a confirmed case of Pertussis in 2009. Natality (Birth) Example: There were 690 infants born with a very low birthweight (less than 1500 grams) in 2009. Rate or percentage of cases in the county Mortality Disease Example: For every 100,000 residents of Maricopa County, 134.7 died due to cancer in 2009.* Communicable Disease Example: For every 100,000 residents of Maricopa County, 1.6 were diagnosed with Pertussis in 2009. Natality (Birth) Example: Among all live births in 2009, 1.2% were born with a very low birthweight. All data in this report are for calendar years 2005 through 2009. Technical Notes Population Statistics Population statistics are shown in Tables 1.01 and 1.02. This report used the most recent U.S. Census data and estimates for population figures at the time of analysis. • The Maricopa County population is from the U.S. Census Bureau, County Population Estimates by Age, Sex, Race and Hispanic Origin for July 1, 2009, released June 10, 2010. This is at http://www.census.gov/popest/counties/asrh/files/cc-est2009-alldata-04.csv The population estimates used in this report are from the U.S. Census and based on the Census 2000 population. The population for a community, such as Maricopa County, changes as a result of births, deaths, and migration (moving into or out of the location). Although it is relatively easy to measure the change in population due to births and deaths, it is more difficult to measure the change due to migration. Population estimates may underestimate or overestimate the true population. Population estimates have the potential to become less reliable as more time passes from the last census count because the actual number of people living in a given area changes each year. Many of the rates shown in this report use population estimates as the denominator (e.g., 50 cases of disease for every 100,000 people) as these are the best population estimates available. Therefore, the true burden of disease within Maricopa County may be understated (if the population estimate is actually larger than the true population) or overstated (if the population estimate is actually smaller than the true population) This report defines “Hispanic” as all respondents listing Hispanic as origin, either alone or in combination with any race. The table below shows how the U.S. Census variables of “origin” and “race” are used for categorization in this report. Using birth certificate data from the National Center for Health Statistics, the race and Hispanic origin for both the mother and father of the baby were converted to the 1997 Office of Management and Budget (OMB) race categories. From the conversion, the race and Hispanic origin of the child was then calculated. Due to this new methodology, an actual increase in the population for Hispanic origin can be observed, specifically in the under 10 years of age population. Maricopa County Health Status Report 2009 Page 41 Mortality (Death) Statistics Mortality (death) statistics are shown in Tables 2.01a through 2.14. Any crude death rate can be calculated by taking the number of deaths from any mortality table in this report and dividing it by the appropriate age, gender, race/ethnicity, or total population denominator found in Tables 1.01 and 1.02. In tables where rates are shown, “0” means there were no cases/deaths in that category. However, when the number “0.0” is shown, it means that there were fewer than .05 deaths per 100,000 residents. Only one condition, Sudden Infant Death Syndrome (SIDS) is calculated differently. By definition, SIDS deaths only occur in infants; as a result, these numbers are divided by the number of live births (rather than the standard 100,000 residents), to produce a rate per 1,000 live births. All data on mortality statistics are from death certificates included in the Arizona Department of Health Services (ADHS) final annual files. The data presented in this report include Maricopa County residents only. This includes Maricopa County residents who died in Maricopa County and residents who died elsewhere. Non-Maricopa County residents who died in Maricopa County are not included. During the course of routine data review, several deaths were identified as being in one county even though their city and ZIP codes indicated that they lived in a different county. For these discrepant cases, final classification in this report was based on the address of the resident. (The address is determined by reviewing the street address, city, county, and ZIP code). Therefore, the numbers presented in this report may not precisely match those presented by ADHS in other reports. In addition, one death was updated with an accurate cause of death. For both mortality and morbidity (communicable disease), rates for some racial/ethnic populations such as American Indians, Asians, and African Americans may be high compared to other groups. Because these groups have relatively small populations in Maricopa County (under 150,000 residents as shown in Tables 1.01 and 1.02), these high rates may represent very few deaths/cases. Among small populations, rates are less stable so one case or death can cause a big increase or decrease in the rates from year to year. Therefore, it is advisable to examine the raw number of cases or deaths, in addition to the rates. Tables 2.01b, 2.02b, 2.03b, 2.04b, 2.06b, and 2.07b do not provide a rate for the categories labeled “Unknown Age” or “Unknown Race/Ethnicity.” This is because there is no denominator to use for calculating the rate as the U.S. Census does not report any data on “unknown age” or “unknown race/ethnicity.” In Tables 2.03a through 2.05b, unintentional injury deaths can be found by adding two categories – “motor vehicle collisions” and “all other accidents and adverse effects.” The total for unintentional injuries in this report is higher than the figure reported by ADHS because deaths from “complications of medical and surgical care” are included in most tables in this report. This convention follows the national model for classifying unintentional injuries. “Complications of medical and surgical care” include deaths from medical devices, adverse effects related to correct doses of therapeutic medications, and misadventures to patients during surgical or medical care (ICD-10 Y40-Y84, Y88). The only exceptions to this method are in Tables 2.13 and 2.14, where unintentional injury deaths do not include complications of medical/surgical care to make them comparable to the Healthy People 2010 goals. Unless otherwise noted, all deaths are attributed to one underlying condition, based on death th certificate information, using the rules of the International Classification of Diseases, 10 Revision (ICD-10). Causes of death are classified by ICD-10 codes. The codes used for each category are listed in Table 5.2 below. Although other causes may have contributed to death, the underlying cause of death is the cause that began the disease process that resulted in death. The underlying causes of death shown in this report are based on the 113 selected causes of death reported by NCHS in their annual report. A few additional causes of death, like coccidioidomycosis, have been selected due to their importance in Arizona. Tables 2.06a through 2.08b show deaths resulting directly (induced) and indirectly (related) from firearms, drugs, and alcohol. Drugs include prescription medications and illegal substances, regardless of whether use was recreational or medicinal. Tobacco is considered a drug by the National Center for Health Statistics (NCHS). Since tobacco is not often considered to be a drug in the colloquial sense, drug-related deaths are listed both with and without tobacco-related deaths. “Induced” deaths refer to cases in which the use of a firearm, drug, or alcohol contributed directly to the cause of death, i.e., death due to an overdose of drugs, or a fatal gunshot injury. “Related” deaths refer to cases in which the use of a drug or alcohol was listed on the death certificate, but the substance was not necessarily involved in the final cause of death, i.e., death due to dehydration resulting from environmental exposure, with chronic drug use noted elsewhere on the certificate. Related causes of deaths are counted using multiple causes of death (MCD) data supplied by ADHS with the final annual mortality files. Maricopa County Health Status Report 2009 Page 44 Firearm, drug, and alcohol induced deaths overlap with a number of other categories, such as suicide, homicide, unintentional injury, etc. For example, a firearm induced death may also be classified as a suicide, homicide, or “other accident or adverse effect.” An alcohol induced death may be attributed to a long or short term effect of alcohol, such as acute alcohol intoxication or alcoholic cardiomyopathy. This report only reports heat deaths where heat is the underlying COD. In other reports produced by Maricopa County regarding heat deaths, the number of deaths may be greater. In the other reports the following criteria is used in classifying heat deaths. Heat caused deaths are death where heat directly caused the decedent to die. The death certificate has heat mentioned in the Part I of the death certificate. Part I is a progression of disease that leads to the immediate cause of death. Heat related deaths are deaths where the heat played a part by exacerbating preexisting conditions, resulting in death. The death certificate has heat mentioned in the Part II of the death certificate. Part II is other significant conditions contributing to death but not resulting in the underlying cause given in Part I. In this report, the criterion used for classifying a death as heat is the underlying cause of death code. These deaths may have been classified as heat caused or heat related in the other reports. Tables 2.13 and 2.14 show age-adjusted rates for Maricopa County. Age-adjusting removes the effect of a disproportionately large young or old population on crude death rates by computing an “adjusted” death rate. An age adjusted rate allows comparisons between different groups, even if one group has more older members than another. A high (unadjusted) death rate for residents of a certain town may be high only because that town has more senior citizens, who are more likely to die than younger residents. Age-adjusted rates allow comparisons between towns, ethnic groups, etc. by adjusting for how many older and younger members there are in the group. For example, the population pyramids at the beginning of this report show a greater percentage of White Non-Hispanics over the age 65 than Hispanics. This creates a much lower crude death rate for Hispanics compared to White Non-Hispanics. Age adjustment shows that White nonHispanics still have a higher death rate, but the difference is not as extreme. Maricopa County rates are age-adjusted to the 2000 standard population using the direct method described in Health, United States, 2002 published by the National Center for Health Statistics at www.cdc.gov/nchs/data/hus/hus02.pdf, p.414. Arizona, Healthy People goals, and U.S. rates included in this report are published as age-adjusted to the 2000 standard population by the agencies generating the rates. For Table 2.14, the mortality rate for breast cancer is calculated among women only (using only female residents in the denominator). The few male residents with breast cancer are not included in the numerator of the rate for breast cancer. For Tables 2.03b, 2.04b, 2.05b, and 2.14, prostate cancer rates are calculated among men only (using only male residents in the denominator), and uterine cancer, ovarian cancer, and pregnancy death rates are calculated among women only. In all other tables, and for all other diseases, the total population is used for the denominator for these conditions. Maricopa County Health Status Report 2009 Page 45 Communicable Disease (Morbidity) Statistics Communicable disease statistics are shown in Tables 3.01a through 3.04. Any morbidity rate except for Streptococcal Group B Invasive disease can be calculated by taking the number of cases from any communicable disease table in this report and dividing it by the appropriate age, gender, race/ethnicity, or total population denominator found in Tables 1.01 and 1.02. Because Streptococcal Group B Invasive disease is only reportable in infants under 90 days, these numbers are divided by the number of live births, to produce a rate per 1,000 live births. The data presented in this report include communicable diseases to residents of Maricopa County. This includes Maricopa County residents who acquired a disease in Maricopa County and residents who acquired a disease elsewhere. Non-Maricopa County residents are not included, even if they acquired a disease in Maricopa County. For Hansen’s disease only, tables include residents from various counties in Arizona, regardless of where the disease was acquired. Other reports may show different morbidity rates because they include non-residents who acquired disease in Maricopa County or they exclude residents who were exposed outside of the county. For example, MCDPH Health Status Reports show nine residents with malaria in 2009. All of this cases acquired malaria outside of Maricopa County. All data on communicable diseases are new cases of reportable diseases from the ADHS final annual files. There are several exceptions: data on hepatitis C, genital herpes, gonorrhea, Chlamydia, syphilis, and Hansen’s disease are from MCDPH data files. Data on tuberculosis, STDs, HIV, and AIDS may differ from other reports published by MCDPH and ADHS. Data on these diseases are in databases that may have been updated during the time between reports. The data for this report were acquired on the following dates. Tuberculosis data were acquired on August 2, 2010. STD data were acquired on July 29, 2010. Syphilis data were acquired on July 29, 2010. HIV/AIDS data for Maricopa County residents were acquired on August 17, 2010. HIV/AIDS data for Arizona residents were acquired on October 1, 2010 Communicable disease data include only confirmed cases, with the exception of West Nile virus, hepatitis B (acute), and Hansen’s disease. West Nile virus and hepatitis B data include confirmed and probable cases. Data for Hansen’s disease includes confirmed, probable, and suspect cases diagnosed in Maricopa County (including diagnoses of non-county residents). All communicable disease statistics in this report are new cases for each specified year (incidence). The exception is Hansen’s disease for which the prevalence (all existing cases from all years) is shown. The figures presented for Hansen’s disease may differ from previous years due to the addition of cases diagnosed in previous years but only recently available. Data can be classified by date of onset, date of diagnosis or date of report to the health department. All diseases/conditions listed in this report are classified by the date of report to the health department. The only exceptions are HIV, AIDS, and Hansen’s disease which are classified by the date of diagnosis. During the course of routine data review, one case was identified to have an incorrect age and sex. A case of Streptococcal Group B Invasive disease was identified to be older than 90 days, and was not included in this report (confirmed cases of Streptococcal Group B Invasive disease only occur in infants under 90 days). Therefore, the numbers presented in this report may not precisely match those presented by ADHS in other reports. The category “Encephalitis Viral” includes St. Louis encephalitis and all other types of viral encephalitis except for West Nile virus. This may differ somewhat from other reports that include only certain types of encephalitides. Maricopa County Health Status Report 2009 Page 46 In previous Health Status Reports, hepatitis B data were also obtained from the MCDPH. This report uses hepatitis B data for confirmed and probable acute cases from ADHS. Comparison of hepatitis B cases in 2008 and 2009 to previous years will show a decrease in the total number of cases. This decrease is most likely due to changes in the investigation of the disease and not due to a change in the communicability of the disease in Maricopa County. In 2009, a major laboratory changed reporting practices for coccidiodomycosis. Due to these changes, a large increase in the total number of cases has been observed. This increase in cases does not mean there is an increase of disease in Maricopa County. Rather, this means that more cases are being reported to public health. A sharp decrease in the number of reported cases of genital herpes occurred in 2009. A review of the cases indicated a large decrease in the number of laboratory reported cases. Although no explanation for this decrease has been identified, it is not believed to be due to an actual decrease in the incidence of disease. In Tables 3.01a through 3.03b, the category labeled “All Others” includes all reportable diseases in the ADHS communicable disease database that are not listed previously on the table. In April 2008, respiratory syncytial virus (RSV) and norovirus were added to the reportable disease lists. With a full year of reports, these diseases have now been added to the list of communicable diseases in this report. In 2009, an influenza epidemic struck Maricopa County and the world, making it the first influenza pandemic in nearly 40 years. Although thousands of cases of disease were reported, among Maricopa County residents, cases are not published in this report. Information regarding influenza is available in the weekly influenza report for Maricopa County found on the Maricopa County Department of Public Health website at http://www.maricopa.gov/PublicHealth/Services/EPI/Reports/influenza.aspx. Natality (Birth) Statistics Natality (birth) statistics are shown in Tables 4.01 through 4.24. The natality percentages for some groups such older and younger maternal age groups (under 15 and over 45) or certain racial/ethnic groups may be high compared to other groups. These groups have relatively small numbers of births in Maricopa County (as shown in Table 4.01). Although percentages may be high, they may represent only one or two cases in a given category. Therefore, it is advisable to check the raw numbers in addition to the rates. All data on birth statistics are from birth certificates included in the ADHS final annual files. The data presented in this report include births to mothers whose county of residence is Maricopa County. This may include births that did not occur in Maricopa County. Also included are some births that occurred outside of Arizona to mothers who reside in Arizona but whose county and city of residence is unknown. National Center for Health Statistics rules state that if a birth does not occur in the state of residency and the county and city are unknown, the city of residency is assigned to the largest city in the state of residency. For these discrepant cases, final classification in this report was based on the address of the resident. (The address is determined by reviewing the street address, city, county, and ZIP code). Therefore, the numbers presented in this report may not precisely match those presented by ADHS in other reports. Infant mortality statistics shown in Table 4.24 are from unlinked data or the number of infant deaths in a given year. An explanation of unlinked infant mortality data is presented below. 1 Adequate prenatal care is based on the Adequacy of Prenatal Care Utilization Index (APCUI) . This index considers both the timing of prenatal care initiation and the number of visits after care was initiated, comparing the number of actual visits to the number of visits recommended by the American College of Obstetricians and Gynecologists. This index does not describe the quality of the care or adjust for maternal risk factors. Only singleton (the birth of one baby rather than twins, triplets, or other multiple births) births are shown in these tables because twins and higher order births are typically higher risk pregnancies, and the index is not appropriate for summarizing their prenatal care usage. a) Inadequate: Prenatal care began after the fourth month of pregnancy or less than 50% of the expected prenatal care visits were attended b) Intermediate: Prenatal care began before the fourth month and 50 to 70% of the expected visits were attended c) Adequate: Prenatal care began prior to the fourth month and 80-109% of expected visits were attended d) Adequate plus: Prenatal care began prior to the fourth month of pregnancy and 110% or more of the expected visits were attended The following definitions related to reproductive health have been established by National Center 2 for Health Statistics : 3 Birth or Live Birth – The complete expulsion or extraction from the mother of a product of human conception, irrespective of the duration of pregnancy or birthweight which, after such expulsion or extraction, breathes or shows any other evidence of life such as beating of the heart, pulsation of the umbilical cord or definite movement of voluntary muscles, whether or not the umbilical cord has been cut or the placenta is attached. Fertility Rate – Number of live births during a calendar year per 1,000 women age 15 to 44. Births to Teenage Mothers − Live births occurring to females under age twenty. Very Low Birthweight – Any neonate weighing less than 1,500 grams at births (less than 3 pounds, 4 ounces). Preterm – Any neonate whose birth occurs through the end of the last day of the 37 th week (259 day), following onset of the last menstrual period. th Infant Death – Any death at any time from birth up to, but not including, the first year of age (<365 days). Infant Mortality Rate – Number of infant deaths per 1,000 live births. Maternal Mortality Rate – Number of deaths to the mother per 100,000 births. Neonatal Mortality Rate – Number of deaths to infants at or under 28 days of age, per 1,000 live births. Postnatal (Postneonatal) Mortality Rate – Number of deaths to infants between 29 days and 365 days after birth, per 1,000 live births. Unlinked Infant Mortality – The number of infant deaths in a given calendar year, as determined from mortality records. Healthy People 2010 Goals The Centers for Disease Control and Prevention provide national health goals for the year 2010 – Healthy People (HP) 2010 goals. Some of these goals are shown in Tables 2.14, 3.04, and 4.16. A hyphen indicates that no goal has been set for that indicator or the goal is not comparable to Maricopa County data. “0” indicates that the goal is zero. This report provides HP 2010 Goals only when the goals are comparable to Maricopa County data. A goal is considered comparable in this report if two conditions exist: First, the HP Goal must include the same conditions as are included in the Maricopa County data. For example, if the Maricopa County figure for heart disease includes all heart disease but the HP 2010 goal includes only coronary heart disease, the goal is not listed as a comparison to the heart disease rate. Second, the HP 2010 goals use ICD-9 codes while the Maricopa County data use ICD-10 codes. Therefore, they may or may not be comparable. For this reason, only select HP 2010 goals are presented here for comparison. A nationally tested “conversion ratio” between ICD-9 and ICD-10 codes for any given condition must fall between certain boundaries. The conversion ratio measures how closely the two classifications resemble each other. A conversion ratio of “1” means that the ICD-9 codes are equivalent to the ICD-10 codes for that category. If the conversion ratio is between 0.95 and 1.05, a direct comparison is made with no mention of any differences. If the ratio does not meet this requirement, but is between 0.80 and 1.20, the potential bias is mentioned in the report, along with specifying the direction of the bias. A ratio that exceeds the .80 to 1.20 range is not used for comparisons.
