Technical Notes 41 Table 3.04 30 Introduction This report provides health status information for the year 2008 for residents of Maricopa County, Arizona. It includes data for mortality (death), communicable diseases, and natality (birth), as well as information on the size and demographic distribution of the population. For each disease or health indicator, the following information is provided: Number of cases in the county Communicable Disease Example: 90 residents of Maricopa County were diagnosed with a confirmed or probable case of West Nile Virus in 2008. Natality (Birth) Example: There were 714 infants born with a very low birthweight (less than 1500 grams) in 2008. Rate or percentage of cases in the county Communicable Disease Example: For every 100,000 residents of Maricopa County, 2.3 were diagnosed with West Nile Virus in 2008. Natality (Birth) Example: Among all live births in 2008, 1.1% were born with a very low birthweight. All data in this report are for calendar year 2008. Technical Notes Population Statistics Population statistics are shown in Tables 1.01 and 1.02. This report used the most recent U.S. Census data and estimates for population figures at the time of analysis. • The Maricopa County population is from the U.S. Census Bureau, County Population Estimates by Age, Sex, Race and Hispanic Origin for July 1, 2008, released May 14, 2009. This is at http://www.census.gov/popest/counties/asrh/files/cc-est2008-alldata-04.csv The population estimates used in this report are from the U.S. Census and based on the Census 2000 population. The population for a community, such as Maricopa County, changes as a result of births, deaths, and migration (moving into or out of the location). Although it is relatively easy to measure the change in population due to births and deaths, it is more difficult to measure the change due to migration. Population estimates may underestimate or overestimate the true population. Population estimates have the potential to become less reliable as more times passes from the last census count due to an accumulation of error. Many of the rates shown in this report use population estimates as the denominator (e.g., 50 cases of disease for every 100,000 people). Therefore, the true burden of disease within Maricopa County may be underestimated (if the population estimate over exaggerates the true population) or overestimated (if the population estimate under exaggerates the true population) due to population estimates even though these are the best estimates available. This report defines “Hispanic” as: all respondents listing Hispanic, either alone or in combination with any race. The table below shows how the U.S. Census variables of “origin” and “race” are used for categorization in this report. From the conversion, the race and Hispanic origin of the child was then calculated. Any crude death rate can be calculated by taking the number of deaths from any mortality table in this report and dividing it by the appropriate age, gender, race/ethnicity, or total population denominator found in Tables 1.01 and 1.02. In tables where rates are shown, “0” means there were no cases/deaths in that category. However, when the number “0.0” is shown, it means that there were fewer than .05 deaths per 100,000 residents. Only one condition, Sudden Infant Death Syndrome (SIDS) is calculated differently. By definition, SIDS deaths only occur in infants; as a result, these numbers are divided by the number of live births (rather than the standard 100,000 residents), to produce a rate per 1,000 live births. All data on mortality statistics are from death certificates included in the Arizona Department of Health Services (ADHS) final annual files. The data presented in this report include Maricopa County residents only. This includes Maricopa County residents who died in Maricopa County and residents who died elsewhere. Non-Maricopa County residents who died in Maricopa County are not included. In January 2008, ADHS Office of Vital Records implemented a new online death registration system. Maricopa County began entering deaths occurring in Maricopa County on July 1, 2008. Due to unforeseen circumstances, a large number of deaths were misclassified to ICD-10 codes of „R00-R99‟ (Symptoms, signs, and abnormal findings), including a large proportion coded to a single code of „R99‟ (Unknown cause of mortality). Unfortunately, this misclassification was not recognized before ADHS published the Arizona Health Status and Vital Statistics, 2008 report. Because Maricopa County published the 2008 health status reports later, this misclassification was corrected and the MCDPH reports are accurate. Therefore, the numbers and rates in Table 2.01a through Table 2.03b will not match the numbers reported in early printed reports by ADHS. ADHS later corrected the causes of death in the online copy of their annual report. Please see http://www.azdhs.gov/plan/report/ahs/ahs2008/pdf/misclassification.pdf for a more detailed explanation of this issue by ADHS Vital Statistician Christopher Mrela. During the course of routine data review, several deaths were identified as being in one county even though their city and ZIP codes indicated that they lived in a different county. For these discrepant cases, final classification in this report was based on the address of the resident. (The address is determined by reviewing the street address, city, county, and ZIP code). Therefore, the numbers presented in this report may not precisely match those presented by ADHS in other reports. In addition, one death was updated with an accurate cause of death. For both mortality and morbidity (communicable disease), rates for some racial/ethnic populations such as American Indians, Asians, and African Americans may be high compared to other groups. Because these groups have relatively small populations in Maricopa County (under 150,000 residents as shown in Tables 1.01 and 1.02), these high rates may represent very few deaths/cases. Among small populations, rates are less stable so one case or death can cause a big increase or decrease in the rates. Therefore, it is advisable to examine the raw number of cases or deaths, in addition to the rates. Tables 2.01b, 2.02b, 2.03b, 2.04b, 2.06b, and 2.07b do not provide a rate for the categories labeled “Unknown Age” or “Unknown Race/Ethnicity.” This is because there is no denominator to use for calculating the rate as the U.S. Census does not report any data on “unknown age” or “unknown race/ethnicity.” In Tables 2.03a through 2.05b, unintentional injury deaths can be found by adding two categories – “motor vehicle collisions” and “all other accidents and adverse effects.” The total for unintentional injuries in this report is higher than the figure reported by ADHS because deaths from “complications of medical and surgical care” are included in this report. This convention follows the national model for classifying unintentional injuries. “Complications of medical and surgical care” include deaths from medical devices, adverse effects related to correct doses of Maricopa County Health Status Report 2008 Page 42 therapeutic medications, and misadventures to patients during surgical or medical care (ICD-10 Y40-Y84, Y88). The only exceptions to this method are in Tables 2.13 and 2.14, where unintentional injury deaths do not include complications of medical/surgical care. This change makes the numbers comparable to Healthy People 2010 goals. Unless otherwise noted, all deaths are attributed to one underlying condition, based on death th certificate information, using the rules of the International Classification of Diseases, 10 Revision (ICD-10). Causes of death are classified by ICD-10 codes. The codes used for each category are listed in Table 5.2 below. Although other causes may have contributed to death, the underlying cause of death is the cause that began the disease process that resulted in death. The underlying causes of death chosen for this report are based on the 113 selected causes of death reported by NCHS in their annual report. A few additional causes of death, like coccidioidomycosis, have been selected due to their importance in Arizona. Firearm, drug, and alcohol induced deaths are also included in a number of other categories, such as suicide, homicide, unintentional injury, etc. For example, a firearm induced death may also be classified as a suicide, homicide, or “other accident or adverse effect.” An alcohol induced death may be attributed to a long or short term effect of alcohol, such as acute alcohol intoxication or alcoholic cardiomyopathy. Tables 2.06a through 2.08b show deaths resulting directly (induced) and indirectly (related) from firearms, drugs, and alcohol. Drugs include prescription medications and illegal substances, Maricopa County Health Status Report 2008 Page 44 regardless of whether use was recreational or medicinal. Tobacco is considered a drug by the National Center for Health Statistics (NCHS). Since tobacco is not often considered to be a drug in the colloquial sense, drug-related deaths are listed both with and without tobacco-related deaths. Induced deaths refer to cases in which the use of a firearm, drug, or alcohol contributed directly to the cause of death, i.e., death due to an overdose of drugs, or a fatal gunshot injury. Related deaths refer to cases in which the use of a drug or alcohol was listed on the death certificate, but the substance was not necessarily involved in the final cause of death, i.e., death due to dehydration resulting from environmental exposure, with chronic drug use noted elsewhere on the certificate. Related causes of deaths are counted using multiple causes of death (MCD) data supplied by ADHS with the final annual mortality files. Tables 2.13 and 2.14 show age-adjusted rates for Maricopa County. Age-adjusting removes the effect of a disproportionately large young or old population on crude death rates by computing an overall death rate based on a population with a standard age distribution. An age adjusted rate allows comparisons between different groups, even if one group has more older members than another. A high (unadjusted) death rate for residents of a certain town may be high only because that town has more elderly members, who are more likely to die than younger members. Ageadjusted rates allow comparisons between towns, ethic groups, etc. by adjusting for how many older and younger members there are in the group. For example, the population pyramids at the beginning of this report show a greater percentage of White Non-Hispanics over the age 65 than Hispanics. This creates a much lower crude death rate for Hispanics compared to White NonHispanics. Age adjustment shows that White non-Hispanics still have a higher death rate, but the difference is not as extreme. Maricopa County rates are age-adjusted to the 2000 standard population using the direct method described in Health, United States, 2002 published by the National Center for Health Statistics at www.cdc.gov/nchs/data/hus/hus02.pdf, p.414. Arizona, Healthy People goals, and U.S. rates included in this report are published as age-adjusted to the 2000 standard population by the agencies generating the rates. For Table 2.14, the mortality rate for breast cancer is calculated among women only (using only female residents in the denominator). For Tables 2.03b, 2.04b, 2.05b, and 2.14, prostate cancer rates are calculated among men only (using only male residents in the denominator), and uterine cancer, ovarian cancer, and pregnancy death rates are calculated among women only. In all other tables, and for all other diseases, the total population is used for the denominator for these conditions. Maricopa County Health Status Report 2008 Page 45 Communicable Disease (Morbidity) Statistics Communicable disease statistics are shown in Tables 3.01a through 3.04. Any morbidity rate except for Streptococcal Group B Invasive disease can be calculated by taking the number of cases from any communicable disease table in this report and dividing it by the appropriate age, gender, race/ethnicity, or total population denominator found in Tables 1.01 and 1.02. Because Streptococcal Group B Invasive disease is only reportable in infants under 90 days, these numbers are divided by the number of live births, to produce a rate per 1,000 live births. The data presented in this report include communicable diseases to residents of Maricopa County. This includes Maricopa County residents who acquired a disease in Maricopa County and residents who acquired a disease elsewhere. Non-Maricopa County residents are not included, even if they acquired a disease in Maricopa County. For Hansen‟s disease only, tables include residents from various counties in Arizona, regardless of where the disease was acquired. Other reports may show different morbidity rates because they include non-residents who acquired disease in Maricopa County or they exclude residents who were exposed outside of the county. For example, MCDPH Health Status Reports show six residents with West Nile virus (WNV) in 2003. Two of these cases acquired WNV outside of Maricopa County; the other four cases were acquired locally. All data on communicable diseases are new cases of reportable diseases from the ADHS final annual files. There are several exceptions: data on hepatitis C, genital herpes, gonorrhea, Chlamydia, syphilis, and Hansen‟s disease are from MCDPH data files. Data on tuberculosis, STDs, HIV, and AIDS may differ from other reports published by MCDPH and ADHS. Data on these diseases are in databases that may have been updated during the time between reports. The data for this report were acquired on the following dates. Tuberculosis data were acquired on July 10, 2009. STD data were acquired on June 25, 2009. Syphilis data were acquired on June 25, 2009. HIV/AIDS data for Maricopa County residents were acquired on July 10, 2009. HIV/AIDS data for Arizona residents were acquired on March 17, 2009. Communicable disease data include only confirmed cases, with the exception of West Nile Virus, hepatitis B (acute), and Hansen‟s disease. West Nile virus and hepatitis B data include confirmed and probable cases. Data for Hansen‟s disease includes confirmed, probable, and suspect cases diagnosed in Maricopa County (including diagnoses of non-county residents). All communicable disease statistics in this report are new cases for each specified year (incidence). The exception is Hansen‟s disease for which the prevalence (all existing cases from all years) is shown. The figures presented for Hansen‟s disease may differ from previous years due to the addition of cases diagnosed in previous years but only recently available. Data can be classified by date of onset, date of diagnosis or date of report to the health department. All diseases/conditions listed in this report are classified by the date of report to the health department. The only exceptions are HIV, AIDS, and Hansen‟s disease which are classified by the date of diagnosis. The category “Encephalitis Viral” includes St. Louis encephalitis and all other types of viral encephalitis except for West Nile virus. This may differ somewhat from other reports that include only certain types of encephalitides. In previous Health Status Reports, hepatitis B data were also obtained from the MCDPH. This report uses hepatitis B data for confirmed and probable acute cases from ADHS. Comparison of hepatitis B cases in 2008 to previous years will show a decrease in the total number of cases. This decrease is most likely due to changes in the investigation of the disease and not due to a change in the communicability of the disease in Maricopa County. Maricopa County Health Status Report 2008 Page 46 In previous Health Status Reports, hepatitis C and vancomycin-resistant enterococci (VRE) cases were reported. While hepatitis C is still reportable, cuts in staff and limited funding have prevented an investigation of the disease. Therefore, with the validity of the data in question, the data is not presented in this report. VRE is not reported in this report because it was removed from the reportable disease lists on April 1, 2008. In Tables 3.01a through 3.03b, the category labeled “All Others” includes all reportable diseases in the ADHS communicable disease database that are not listed previously on the table. In addition to VRE being removed from the reportable disease lists, several diseases were also added. This includes respiratory syncytial virus (RSV) and norovirus, which can be found in the counts for „All Others‟. The addition of these two diseases partially explains a sharp increase in the total count for „All Others‟ compared to earlier years. Natality (Birth) Statistics Natality (birth) statistics are shown in Tables 4.01 through 4.24. The natality percentages for some groups such older and younger maternal age groups (under 15 and over 45) or certain racial/ethnic groups may be high compared to other groups. These groups have relatively small numbers of births in Maricopa County (as shown in Table 4.01). Although percentages may be high, they may represent only one or two cases in a given category. Therefore, it is advisable to check the raw numbers in addition to the rates. All data on birth statistics are from birth certificates included in the Arizona Department of Health Services (ADHS) final annual files. The data presented in this report include births to mother‟s whose county of residence is Maricopa County. This may include births that did not occur in Maricopa County. Also included are some births that occurred outside of Arizona to mothers who reside in Arizona but the county and city of residence is unknown. National Center for Health Statistics rules state that if a birth does not occur in the state of residency and the county and city are unknown, the city of residency is assigned to the largest city in the state of residency. An explanation of unlinked infant mortality data is presented below. 1 Adequate prenatal care is based on the Adequacy of Prenatal Care Utilization Index (APCUI) . This index considers both the timing of prenatal care initiation and the number of visits after care was initiated, comparing the number of actual visits to the number of visits recommended by the American College of Obstetricians and Gynecologists. This index does not describe the quality of the care or adjust for maternal risk factors. Only singleton (the birth of one baby rather than twins, triplets, or other multiple births) births are shown in these tables because twins and higher order births are typically higher risk pregnancies, and the index is not appropriate for summarizing their prenatal care usage. a) Inadequate: Prenatal care began after the fourth month of pregnancy or less than 50% of the expected prenatal care visits were attended b) Intermediate: Prenatal care began before the fourth month and 50 to 70% of the expected visits were attended c) Adequate: Prenatal care began prior to the fourth month and 80-109% of expected visits were attended d) Adequate plus: Prenatal care began prior to the fourth month of pregnancy and 110% or more of the expected visits were attended The following definitions related to reproductive health have been established by National Center 2 for Health Statistics : 3 Birth or Live Birth – The complete expulsion or extraction from the mother of a product of human conception, irrespective of the duration of pregnancy or birthweight which, after such expulsion or extraction, breathes or shows any other evidence of life such as beating of the heart, pulsation of the umbilical cord or definite movement of voluntary muscles, whether or not the umbilical cord has been cut or the placenta is attached. Fertility Rate – Number of live births during a calendar year per 1,000 women age 15 to 44. Births to Teenage Mothers − Live births occurring to females under age twenty. Birth Weight – The weight of a neonate determined immediately after delivery or as soon thereafter as possible. Low Birthweight – Any neonate weighing less than 2,500 grams at birth (less than 5 pounds 8 ounces). Very Low Birthweight – Any neonate weighing less than 1,500 grams at births (less than 3 pounds, 4 ounces). Neonatal Mortality Rate – Number of deaths to infants at or under 28 days of age, per 1,000 live births. Postnatal (Postneonatal) Mortality Rate – Number of deaths to infants between 29 days and 365 days after birth, per 1,000 live births. Unlinked Infant Mortality – The number of infant deaths in a given calendar year, as determined from mortality records. Healthy People 2010 Goals The Centers for Disease Control and Prevention provide national health goals for the year 2010 – Healthy People (HP) 2010 goals. Some of these goals are shown in Tables 2.14, 3.04, and 4.16. A hyphen indicates that no goal has been set for that indicator or the goal is not comparable to Maricopa County data. “0” indicates that the goal is zero. This report provides HP 2010 Goals only when the goals are comparable to Maricopa County data. A goal is considered comparable in this report if two conditions exist: First, the HP Goal must include the same conditions as are included in the Maricopa County data. For example, if the Maricopa County figure for heart disease includes all heart disease but the HP 2010 goal includes only coronary heart disease, the goal is not listed as a comparison to the heart disease rate. Second, the HP 2010 goals use ICD-9 codes while the Maricopa County 2007 data use ICD-10 codes. Therefore, they may or may not be comparable. For this reason, only select HP 2010 goals are presented here for comparison. A nationally tested “conversion ratio” between ICD-9 and ICD-10 codes for any given condition must fall between certain boundaries. The conversion ratio measures how closely the two classifications resemble each other. A conversion ratio of “1” means that the ICD-9 codes are equivalent to the ICD-10 codes for that category. If the conversion ratio is between 0.95 and 1.05, a direct comparison is made with no mention of any differences. If the ratio does not meet this requirement, but is between 0.80 and 1.20, the potential bias is mentioned in the report, along with specifying the direction of the bias. A ratio that exceeds the .80 to 1.20 range is not used for comparisons.
