It includes data for morbidity (death), communicable diseases, and natality (birth), as well as information on the size and demographic distribution of the population. For each disease or health indicator, the following information is provided: • Number of cases in the county • • • Communicable Disease Example: 248 residents of Maricopa County were diagnosed with confirmed cases of Pertussis (whooping cough) in 2005. Natality (Birth) Example: There were 735 infants born with a very low birthweight (less then 1500 grams) in 2005. Rate or percentage of cases in the county • • Communicable Disease Example: For every 100,000 residents of Maricopa County, 6.8 were diagnosed with Pertussis in 2005. Natality (Birth) Example: Among all live births in 2005, 1.2% were born with a very low birthweight. All data in this report are for calendar year 2005. Any crude death rate can be calculated by taking the number of deaths from any mortality table in this report and dividing it by the appropriate age, gender, race/ethnicity, or total population denominator found in Tables 1.01 and 1.02. In tables where rates are shown, “0” means there were no cases/deaths in that category. However, when the number “0.0” is shown, it means that there were fewer than .05 deaths per 100,000 residents. All data on mortality statistics are from the Arizona Department of Health Services (ADHS) final annual files. The Maricopa County mortality data are from death certificates for Maricopa County residents only. This includes Maricopa County residents who died in Maricopa County and residents who died elsewhere. Non-Maricopa County residents who died in Maricopa County are not included. In the course of routine data cleaning, duplicate mortality records were identified. Five duplicated records have been deleted from the mortality records in 2005. As a result of these deletions, the numbers presented in this report may not match those presented by ADHS for Maricopa County. For both mortality and morbidity (communicable disease), rates for some racial/ethnic populations such as American Indians, Asians, and African Americans are high compared to other groups. Because these groups have relatively small populations in Maricopa County (under 150,000 residents as shown in Tables 1.01 and 1.02), these high rates may represent very few deaths/cases. Among small populations, rates are less stable so one case or death can cause a big increase or decrease in the rates. Therefore, it is advisable to examine the raw number of cases or deaths, in addition to the rates. In Tables 2.03a through 2.05b, unintentional injury deaths can be found by adding two categories – “motor vehicle collisions” and “all other accidents and adverse effects.” The total for unintentional injuries in this report is higher than the figure reported by ADHS because deaths from “complications of medical and surgical care” are included in this report. This convention follows the national model for classifying unintentional injuries. “Complications of medical and surgical care” include deaths from medical devices, adverse effects related to correct doses of therapeutic medications, and misadventures to patients during surgical or medical care (ICD-10 Y40-Y84, Y88). The only exceptions to this method are in Tables 2.13 and 2.14, where unintentional injury deaths do not include complications of medical/surgical care. This change makes the numbers comparable to Healthy People 2010 goals. Tables 2.01b, 2.02b, 2.03b, 2.04b, 2.06b, and 2.07b do not provide a rate for the categories labeled “Unknown Age” or “Unknown Race/Ethnicity.” This is because there is no denominator to use for calculating the rate as the U.S. Census does not report any data on “unknown age” or “unknown race/ethnicity.” Unless otherwise noted, all deaths are attributed to one underlying condition, based on death certificate information, using the rules of the International Classification of Diseases. Causes of death are classified by ICD-10 codes. The codes used for each category are listed in Table 5.2 below. Firearm, drug, and alcohol induced deaths are also included in a number of other categories, such as suicide, homicide, unintentional injury, etc. For example, a firearm induced death may also be classified as a suicide, homicide, or “other accident or adverse effect.” An alcohol induced death may be attributed to a long or short term effect of alcohol, such as acute alcohol intoxication or alcoholic cardiomyopathy. Tables 2.06a through 2.08b show deaths resulting directly (induced) and indirectly (related) from firearms, drugs, and alcohol. Drugs include prescription medications and illegal substances, regardless of whether use was recreational or medicinal. Tobacco is considered a drug by the Maricopa County Health Status Report 2005 Page 38 National Center for Health Statistics (NCHS). Since tobacco is not often considered to be a drug in the colloquial sense, drug-related deaths are listed both with and without tobacco-related deaths. Induced deaths refer to cases in which the use of a firearm, drug, or alcohol contributed directly to the cause of death, i.e., death due to an overdose of drugs, or a fatal gunshot injury. Related deaths refer to cases in which the use of a drug or alcohol was listed on the death certificate, but the substance was not necessarily involved in the final cause of death, i.e., death due to dehydration resulting from environmental exposure, with chronic drug use noted elsewhere on the certificate. Related causes of deaths are counted using multiple cause of death (MCD) data supplied by ADHS with the final annual mortality files. Age-adjusting removes the effect of a disproportionately large young or old population on crude death rates by computing an overall death rate based on a population with a standard age distribution. Maricopa County rates are age-adjusted to the 2000 standard population using the direct method described in Health, United States, 2002 published by the National Center for Health Statistics at www.cdc.gov/nchs/data/hus/hus02.pdf, p.414. Arizona, Healthy People goals, and U.S. rates included in this report are published as age-adjusted to the 2000 standard population by the agencies generating the rates. Communicable Disease (Morbidity) Statistics Communicable disease statistics are shown in Tables 3.01a through 3.04. Any morbidity rate except for Streptococcal Group B Invasive disease can be calculated by taking the number of cases from any communicable disease table in this report and dividing it by the appropriate age, gender, race/ethnicity, or total population denominator found in Tables 1.01 and 1.02. Because Streptococcal Group B Invasive disease is only reportable in infants under 90 days, these numbers are divided by the number of live births, to produce a rate per 1,000 live births. The Maricopa County morbidity data are for Maricopa County residents only. This includes Maricopa County residents who acquired a disease in Maricopa County and residents who acquired a disease elsewhere. Non-Maricopa County residents are not included, even if they acquired a disease in Maricopa County. Other reports may show different morbidity rates because they include non-residents who acquired disease in Maricopa County or they exclude residents who were exposed outside of the county. For example, MCDPH Health Status Reports show six residents with West Nile virus (WNV) in 2003. Two of these cases acquired WNV outside of Maricopa County, the other four cases were acquired locally. Other reports and press releases may focus on only the locally acquired WNV cases, regardless of the residency status of the cases. For Hansen’s disease only, tables include residents from various counties in Arizona, regardless of where the disease was acquired. Communicable disease data are from the ADHS final annual files with several exceptions: data on West Nile virus, hepatitis C, genital herpes, gonorrhea, chlamydia, syphilis, and Hansen’s disease are from MCDPH. Because the MCDPH communicable disease database is continuously updated, the data for hepatitis C and West Nile virus can change somewhat over time. Hepatitis C data for 2005 used in this report were generated on October 31, 2006 for confirmed, acute and chronic, physician-reported cases among Maricopa County residents. West Nile virus data for 2005 used in this report were generated on November 15, 2006 for all confirmed cases among Maricopa County residents. Maricopa County Health Status Report 2005 Page 41 In previous Health Status Reports, hepatitis B data were also obtained from the MCDPH. However, there have been a number of issues contributing to unreliable Hepatitis B data. Therefore, this report uses hepatitis B data for acute cases from ADHS. Data on tuberculosis, STDs, HIV, and AIDS may differ from other reports published by MCDPH and ADHS. Data on these diseases are in databases that may have been updated during the time between reports. HIV/AIDS data for Maricopa County are from October 27, 2006 and were provided to MCDPH for this report. Primary and secondary syphilis is shown as “Syphilis-Prim. and Sec”. The “Syphilis - All others” category includes latent and congenital syphilis. These two categories can be added to determine the total number of syphilis cases. Syphilis data were generated on February 23, 2006. Morbidity data include only confirmed cases, with the exception of Hansen’s disease. Data for Hansen’s disease includes all confirmed, suspect, and probable cases diagnosed in Maricopa County, including diagnoses to non-county residents. All communicable disease statistics in this report are new cases for each specified year (incidence). The only exception is Hansen’s disease for which the prevalence (all existing cases from all years) is shown. The figures presented for Hansen’s disease differ from previous years due to the addition of several cases diagnosed in previous years but only recently available. Morbidity data for hepatitis C and West Nile Virus are classified by the earliest date in the record. The earliest date may be date of onset, date of diagnosis, or date of report. HIV, AIDS, and Hansen’s disease data are classified by date of diagnosis. All other data are classified by date of report. The category “Encephalitis Viral” includes St. Louis encephalitis and all other types of viral encephalitis except for West Nile virus. This may differ somewhat from other reports that include only certain types of encephalitides. In Tables 3.01a through 3.03b, the category labeled “All Others” includes all reportable diseases in the ADHS communicable disease database that are not listed previously on the table. Natality (Birth) Statistics Natality (birth) statistics are shown in Tables 4.01 through 4.16. In the course of routine data cleaning, duplicate natality records were identified. One duplicated record has been deleted from the natality records in 2005. As a result of this deletion, the numbers presented in this report may not match those presented by ADHS for Maricopa County. Infant mortality statistics shown in Table 4.16 are from unlinked data as indicated, and the table shows comparisons to Arizona and the U.S. U.S. data are preliminary for 2004, from the National Center for Health Statistics. An explanation of unlinked infant mortality data is presented below. The natality percentages for older and younger maternal age groups (under 15 and over 45) may be high compared to other groups. These groups have relatively small populations in Maricopa County. Although percentages may be high, they may represent only one or two cases in a given category. Therefore, it is advisable to check the raw numbers. Adequate prenatal care is based on the Adequacy of Prenatal Care Utilization Index (APCUI) 1 . This index considers both the timing of prenatal care initiation and the number of visits after care was initiated, comparing the number of actual visits to the number of visits recommended by the American College of Obstetricians and Gynecologists. This index does not describe the quality of the care or adjust for maternal risk factors. Only singleton births are shown in these tables because twins and higher order births are typically higher risk pregnancies, and the index is not appropriate for summarizing their prenatal care usage. a) Inadequate: Prenatal care began after the fourth month of pregnancy or less than 50% of the expected prenatal care visits were attended b) Intermediate: Prenatal care began before the fourth month and 50 to 70% of the expected visits were attended c) Adequate: Prenatal care began prior to the fourth month and 80-109% of expected visits were attended d) Adequate plus: Prenatal care began prior to the fourth month of pregnancy and 110% or more of the expected visits were attended The following definitions related to reproductive health have been established by NCHS 2 : • Birth or Live Birth – The complete expulsion or extraction from the mother of a product of human conception, irrespective of the duration of pregnancy or birthweight which, after such expulsion or extraction, breathes or shows any other evidence of life such as beating of the heart, pulsation of the umbilical cord or definite movement of voluntary muscles, whether or not the umbilical cord has been cut or the placenta is attached. • Birth Rate – Number of live births during a calendar year per 1,000 population. • Births to Teenage Mothers − Live births occurring to females under age twenty. • Birth Weight – The weight of a neonate determined immediately after delivery or as soon thereafter as possible. • Low Birthweight – Any neonate weighing less than 2,500 grams at birth (less than 5 pounds 8 ounces). • Very Low Birthweight – Any neonate weighing less than 1,500 grams at births (less than 3 pounds, 4 ounces). • Preterm – Any neonate whose birth occurs through the end of the last day of the 37th week (259th day), following onset of the last menstrual period. • Infant Death – Any death at any time from birth up to, but not including, the first year of age (<365 days). • Infant Mortality Rate – Number of infant deaths per 1,000 live births. 1 Kotelchuck, M. Maricopa County Health Status Report 2005 Page 43 • Neonatal Mortality Rate – Number of deaths to infants under 28 days of age, per 1,000 live births. • Postnatal (Postneonatal) Mortality Rate – Number of deaths to infants between 28 days and 365 days after birth, per 1,000 live births. • Unlinked Infant Mortality – The number of infant deaths in a given calendar year, as determined from mortality records. • Linked Infant Mortality – Whenever possible, infant death certificate data are linked to birth certificate information to determine parental, demographic and birth data. Not all infant deaths can be linked (up to 3% are lost to follow-up). Healthy People 2010 Goals The Centers for Disease Control and Prevention provide national health goals for the year 2010 – Healthy People (HP) 2010 goals. Some of these goals are shown in Tables 2.14, 3.04, and 4.16. A hyphen indicates that no goal has been set for that indicator or the goal is not comparable to Maricopa County data. “0” indicates that the goal is zero. This report provides HP 2010 Goals only when the goals are comparable to Maricopa County data. A goal is considered comparable in this report if two conditions exist: • First, the HP Goal must include the same conditions as are included in the Maricopa County data. For example, if the Maricopa County figure for heart disease includes all heart disease but the HP 2010 goal includes only coronary heart disease, the goal is not listed as a comparison to the heart disease rate. • Second, the nationally tested “conversion ratio” between ICD-9 and ICD-10 codes for any given condition must fall between certain boundaries. The conversion ratio measures how closely the two classifications resemble each other. A conversion ratio of “1” means that the ICD-9 codes are equivalent to the ICD-10 codes for that category. Because the HP 2010 goals use ICD-9 codes and the Maricopa County 2005 data use ICD-10 codes, they may or may not be comparable. Therefore, if the conversion ratio is between 0.95 and 1.05, a direct comparison is made with no mention of any differences. If the ratio does not meet this requirement, but is between 0.80 and 1.20, the potential bias is mentioned in the report, along with specifying the direction of the bias. A ratio that exceeds the .80 to 1.20 range is not used for comparisons.
