This edition of the Arizona State Board of Nursing Regulatory Journal will report the third survey results of the Commitment to Ongoing Regulatory Excellence (CORE) project. This work began approximately 10 years ago when the National Council State Boards of Nursing (NCSBN) Board of Directors appointed a project advisory group in 1998 to partner with the Urban Institute to provide the oversight and guide development of an ongoing Board of Nursing performance measurement and benchmarking system. The collection of data, research and analysis of quantitative and empirical data has assisted boards in carrying out their public mandate to protect the public by isolating weaknesses and identifying strengths. The 2007 evidence or data assisted over 34 participating boards of nursing to make connections between performance differences and best practices. Examples of metrics deemed essential to understand the operational health of the board of nursing in fulfilling its public protection mission are: • Staff productivity (i.e. number of applicants licensed/investigative cases processed/educational programs approved) • Cost effectiveness (i.e. expense calculations per licensee) • Cycle times (i.e. processing of licensure applications and investigations) Stakeholder satisfaction and perceptions are also critical to understanding the operational health of the regulatory board. The ratings and opened ended questions provided information to direct efforts in gaps in service. The CORE measurement categories are reported as a “dashboard of measures” for the evidenced-based regulation framework. Similar to a dashboard in an airplane, which provides the pilot an overview of the critical functions of the airplane, a Board of Nursing dashboard provides a high level of understanding of how the board is performing. In addition to these performance measures, the Board wants to know how to improve outcomes in better protecting the public. The Arizona State Board of Nursing finds the connection between the numbers and regulatory practices that drive performance is a way to achieve this. These data have been incredibly helpful for Board members and Board staff in meeting the recent proposed agency reduction in response to the State of Arizona $1.5 billion budget deficit. Numerous options are being reviewed by the Governor’s staff and legislature and will continue to be reviewed as the Board works with State of Arizona. To be sure, the agency was running on a very tight budget and had very little if any areas that could be considered “easily expended.” Most of all, the proposed reduction in Board staff has further impacted the already challenging processes of moving cases effectively to protect the public. There is never a good time to reduce staff, and the current economic shortfalls make this even more challenging and discouraging. No decisions will be made without careful consideration of the evidence to support the decision, the potential risk to the public, and the welfare of all who serve the Board. Perhaps, as we struggle with this challenge, it is a time to resonate with our namesake Phoenix and think about how we can turn this into an opportunity. Perhaps we will be forced to think differently, more effectively, and ultimately rise from this challenge, similar to the Phoenix rising from the ashes. At this important time, all ideas are encouraged and welcomed. This change was made to assist in the budgeting challenges that the state is facing and to also avoid licensure fee 14 increases this year. Nurses may renew their license up to six months in advance. RN/ LPN’s due for renewal April 1, 2010, can begin applying in October 2009. Please apply early to avoid delays in licensure. Licenses continue to be for four years. Nurses who do not renew and continue to work on their license after May 1 will accrue a late fee. Documentation is being accepted now and anytime prior to the required renewal date to avoid delays in the licensure processing time. An exception exists for those who have submitted a time-limited document, as they still a need to resubmit a lawful presence document when the prior document expires. Please be aware: A significant number of nurses who renewed online in 2008 also forgot to submit their documentation of citizenship/ lawful presence. Their renewal license was never issued because all of the requirements of the law were not met. Those nurses’ licenses lapsed. When these nurses renew in the future, and if they worked on an expired license, there will be a late fee and as well as an administrative fee for working on an expired license. Thanks to those employers who have facilitated the submission of the citizenship/ lawful presence documentation in their facilities by collecting the “cover sheet” and copy of the citizenship/lawful presence documentation from all nursing staff and submitting the information at one time. The completed “cover sheet” is required to ensure the scanned documents merge with the correct license/certificate holder in the data base. The “cover sheet” can be found on page 21. There are two lists that identify the specific citizenship/lawful presence documents that can be accepted. Please see List A and List B on pages 18. The lists of accepted documents can also be found on our Web page at www.azbn.gov under the Licensure and Certification tab; Applications. The most frequently submitted documents include: a birth certificate, or a U.S. passport. The passport can be expired and still be accepted. Thank you! arizona STATE BOARD OF NURSING Regulatory Journal 15 EDUCATION CORNER By Pamela Randolph RN, MS Associate Director Education and Evidence Based Regulation CORE REPORT—EDUCATION The Commitment to Ongoing Regulatory Excellence (CORE) project under the direction of the National Council of State Boards of Nursing provides nursing regulatory boards with data from both internal and external stakeholders to identify best practices and use benchmarking strategies for Boards of Nursing. The report is comprehensive, involving all Board functions including education and the approval of nursing programs. In 2007, 50 survey tools were sent to nursing programs in AZ with 19 being returned (38% return rate). 32 percent of responding programs reported that they were being monitored for compliance. One thousand survey tools were also were mailed to nurses with 321 returned for a 27 percent return rate. In addition to other items regarding Board functions, nurses were asked to rate the adequacy of their basic nursing education. One hundred survey tools were mailed to employers with 20 returned for a response rate of 20 percent. Employers, were asked to rate the preparedness of newly licensed nurses. Perception of Basic Education and New Graduates Practicing nurses were asked to rate how well their basic education prepared them to provide safe, effective nursing care. The vast majority of responding AZ RNs thought their education prepared them “very well” or “well.” There is a decrease in nurses rating their education in the highest category in both 2005 and 2007, which is thought to be due to experience level of respondents. Similar trends were seen in LPN data. Seven-five percent of nurses who practiced 35 years or more rated their program as preparing them very well, in contrast to 24 percent of nurses with zero to one years of practice. Employers were asked to rate the preparedness of newly licensed nurses to practice on a scale of 1 (very well prepared) to four (very poorly prepared). Respondent employers rated new grads between 2.36 and 2.88 on all functions falling between the “well prepared” and “poorly prepared” categories. However, for AZ graduates, all but two scores are above the national average and all but three scores improved. Scores below the national average, were “administer medications by common routes” and “work effectively within a health care team.” These two areas combined with “perform psychomotor skills” also represent the scores that failed to show improvement. Interestingly, despite being 16 arizona STATE BOARD OF NURSING Regulatory Journal programs informed (3.8 of a possible 4) and assistance provided by Board staff (3.9 of a possible 4). Eighty-four percent of program respondents rated the Board as having adequate regulation of nursing programs, with 10 percent of respondents opining that the Board over-regulated nursing programs, while 5 percent thought the Board under-regulated nursing programs. Summary The Board was rated favorably in most categories by education programs responding to the survey. Nursing program graduates were rated higher than the national average when compared to graduates from other participating states and showed improvements in several areas of functioning. However, the differences are very small in many areas and may be due to natural variation from survey to survey. This factor, combined with low response rates, indicate that the results should be interpreted with caution. Other high-ranking skills included: “communicate relevant information,” “recognize abnormal physical findings,” “teach patients,”and “assess effectiveness of treatments.” Two of these: “teach patients” and “assess effectiveness of treatments,” were among the lowest ranked in 2005. The lowest rankings in 2007 were for: “supervising care provided by others,” “recognizing abnormal diagnostic lab findings” and “documenting a legally defensible account of care.” Despite their low ranking, these functions improved when compared to 2005. Perceptions of Board Functions by Nursing Programs Nursing programs rated the effectiveness of Board functions as very effective in the area of public protection, promotion of quality in nursing programs, and responsiveness to innovation in education (all were above 3.5 on a 4.0 scale). The helpfulness of the Board on educational issues was rated highly (3.8 of a possible 4.0). By accessing www.azbn.gov and clicking on My Services the following options are offered: • Renew active license or certificate online If the renewal process is completed online and proof of citizenship/lawful presence documentation has already been received by the Board, the renewal will be issued within 1-2 days. When paper applications are received in office they are placed in date order received to be processed. Processing can take an average of one to three weeks dependent on the volume of renewals received throughout the renewal cycle. It is also the staff experience that paper applications are more likely to have incomplete information and therefore require the issuance of a deficiency notice, which extends the time it takes to renew the license. When renewing online all required information/data elements must be provided or the applicant cannot proceed. • Place a license or certificate on inactive status online When inactive status is requested online, the license will immediately be placed inactive. If the request is submitted through the mail, the processing of the Inactive Status Application may take up to two weeks based on volume of requests. • Update changes to home, mailing or business address by Jennifer McWilliams RN/LPN Licensing Tech When completed online, changes are made immediately or as soon as the information is added. If a paper form is submitted changes can take an average of two weeks. • Update changes to email address online Email addresses need to be updated when the email address changes, similar to changing a home/mailing address. Knowing the email address allows the Board to send out reminders when the license or certificate is not renewed and before the expiration. • Order a duplicate license or certificate online If requesting a duplicate license or certificate by submitting the hard copy duplicate request form, the form is completed in date order received. The duplicate request can take between one to two weeks to be completed. However if the duplicate is ordered using the website, it goes into the print queue automatically and is printed and mailed within a few days. By Judy Bontrager, RN, MN Associate Director Operations/Licensing Implementation New Telephone System Every two years the Board receives results of the Commitment to Ongoing Regulatory Excellence (CORE) report, conducted by the National Council State Board of Nursing. The purpose of this report is to establish a performance measurement system that incorporates data collection from nurses, employers, and boards of nursing. The Board’s telephone system rated 2 with 1=Excellent, 2=Good, 3=Fair and 4=Poor. 26 The aggregate rating for all the reporting Boards of Nursing was 2.04. Advantages of the new phone system implemented in June 2009 are: 1. Ability of the caller to hear the estimated wait time while on hold. 2. Ability for pertinent board updates, such as the change in the renewal date on RN/LPN licenses, to be heard by the caller while on hold. 3.
